Fundraising Request Form

All Fundraisers at UUS must be reviewed by the Finance Committee and approved by the Board of Trustees. A Fundraising Request form (PDF) must be completed and submitted to [email protected] two weeks in advance of the fundraiser start date (later submissions will be reviewed in special circumstances).
