Advance Care Planning
Our congregation is proud to partner with Honoring Your Wishes: A Community-Wide Advance Care Planning Initiative based in Iowa City. Advance care planning is a process for individuals to:
Discuss goals for future healthcare decisions in the context of their values and beliefs
Produce or update a written plan (i.e., an advance directive) that accurately represents their preferences and prepares others to make healthcare decisions consistent with these preferences.
Certified advance care plan (ACP) facilitators offer confidential individual sessions at no cost. Emma Barnum can notarize the ACP and file a secure copy at UUS. If you would like to develop an ACP, one of the facilitators will contact you.
